During this rotation I had the privilege of working with Dr. Nicola Kotchev, a geriatrician. He delivered daily lectures on the geriatric population about a wide variety of topics, including geriatric assessment, dementia, pressure ulcers, pain management in the elderly, Parkinson’s, medication reconciliation, cognitive assessment, gait, driving in the elderly, advanced directives, and liver disease. I performed physical exams alongside Dr. Kotchev. During my exams Dr. Kotchev guided me on how I can improve my physical diagnosis skills. For example, I had trouble finding pulses on the feet. Dr. Kotchev told me to use the tips of my fingers and not to press down too hard. He also recommended I feel over the bone. Dr. Kotchev also has a background as an orthopedic surgeon. This allowed him to teach me a lot about orthopedics and to help me improve on my musculoskeletal exams. I looked at many x-rays, CT scans and MRIs and have improved on reading them. I also practiced interpreting lab results. I became significantly more proactive during this rotation, which I plan to keep doing as I go on to my future rotations. I improved on my phlebotomy skills, giving intramuscular injections and performing EKGs. I performed many exams on feet, which is something I had not seen in other rotations. I performed neurological exams on many patients, but I can still use practice with this. I have been and will continue to work to improve my neurological exams. One of the experiences I really enjoyed in this rotation was writing my own student note in epic. This gave me the opportunity to understand what it is like to write a note in EMR and to get valuable feedback from my preceptor. I watched the resident present to the attending many times, which has helped me to improve on my own presentations. I would like to continue to improve on my presentations. I plan to do this by asking to do more presentations in my upcoming rotations. Overall, I learned a lot about medicine and geriatrics during my time at this rotation. It was an excellent and fulfilling experience.
