Site Visit Summary:

At my second site visit I presented the following case.

 A 35 y/o Asian Female G1P0 @ 36w 3d gestational age with estimated date of delivery of 3/15/21, dated by LMP 36 weeks. Patient confirms LMP is correct. Patient sent from ATU after NST with ctx q 1.5-3 minutes. The patient was a late registrant and had her first fetal ultrasound on the day of the current visit. The estimated fetal weight is 1,890 g (<1%).  Which based on the estimated age of the fetus is a severe case of IUGR. However it is possible that the real age of the fetus is 32 weeks. Due to lack of compliance the age of the fetus is unknown. The patient was admitted for preterm labor management and monitoring of intrauterine growth restriction. Prenatal labs CBC, Type and Screen, Syphilis and GBS were performed. The patient was given Ampicillin for GBS unknown, betamethasone for lung maturity, Terbuatine as a tocolytic, IV fluids for the contractions and fetal heart rate monitoring.

This patient is from an underserved population and has not had prenatal care. I have learned about some of the challenges this community experiences. Additionally, during my first site visit I didn’t know how to present a case. Professor Melendez spent time explaining to me how to present the case. This time I went through the HPI, the relevant exam and the assessment and plan. I also explained why each part of the plan was necessary. I felt like I improved significantly between the cases that I presented. My presentation was clearer and more organized.
